Glaucoma patients who sleep with their heads elevated on two pillows experience significantly higher intraocular pressure (IOP) compared to lying flat, according to a 2026 study. This elevation compresses neck veins, restricts blood drainage from the head, and reduces ocular perfusion pressure — directly worsening the optic nerve damage that defines the disease.

The mechanics of pillow-induced pressure

Researchers at Zhejiang University enrolled 144 glaucoma patients and measured IOP in both the supine (flat) position and the high-pillow position, defined as 20–35 degrees of head elevation using two regular-sized pillows. Mean IOP rose from 16.62 mm Hg flat to 17.42 mm Hg with pillows, with two-thirds of patients showing an increase averaging 1.61 mm Hg. Twenty-four-hour IOP fluctuation also increased, while ocular perfusion pressure dropped from 58.71 mm Hg to 54.57 mm Hg. Ultrasonography on healthy volunteers confirmed the mechanism: high-pillow posture constricted both internal and external jugular veins and accelerated blood flow velocity in the internal jugular.

The authors linked these changes to neck flexion that impedes venous return and aqueous humor outflow.

Glaucoma defined and why nightly posture matters

Standard treatments for glaucoma lower IOP through drops, lasers, or surgery, yet prevention and daily management remain critical. Nighttime spikes and fluctuations accelerate damage. The study reveals that a habit many adopt for comfort or acid reflux — stacking pillows — may counteract those efforts by mechanically raising pressure for hours each night.

Expert reactions stress personalization, not panic

Dr. William Lu, medical director at Dreem Health and uninvolved in the research, told Fox News Digital the findings are "interesting and important" but called for moderation. "It doesn't mean pillows are inherently harmful — it's more about how they're used and the degree of elevation."

Dr. Saema Tahir, a board-certified sleep disorder specialist, added that elevating the head of the bed itself can reduce eye pressure, "but using multiple pillows may not have the same effect — and could even be counterproductive." Beyond glaucoma, improper pillow height disrupts cervical spine alignment. Back sleepers often benefit from minimal or no pillow to keep the neck neutral. Side sleepers typically require enough support to prevent the head from tilting downward, which strains muscles and contributes to chronic pain, headaches, and airway collapse linked to snoring or sleep apnea. These same misalignments can worsen acid reflux when the head is not properly elevated relative to the torso.

Practical adjustments for long-term protection

  • Back sleepers: Test minimal or no pillow to maintain neutral neck position.

  • Side sleepers: Use a single, properly contoured pillow that supports the head without lifting it excessively.

  • Glaucoma patients: Consider raising the entire head of the bed (wedge or bed risers) instead of stacking pillows, which avoids sharp neck flexion.

  • General rule: Prioritize spinal alignment over arbitrary elevation.

Small changes in sleep setup cost little yet accumulate nightly. Patients with diagnosed glaucoma or elevated IOP risk should consult an ophthalmologist or sleep specialist to tailor posture without trading one problem for neck pain or disrupted sleep.

The study does not demand that everyone sleep pillow-free. It exposes how an everyday habit quietly influences a leading cause of irreversible blindness. For those already fighting optic nerve damage, lowering the head a few inches at night may represent one of the simplest defenses available — a quiet reclamation of control over a process long assumed to happen only in the doctor's office.
